NABR Cares Inc. is a non-profit organization dedicated to improving the health and well-being of underserved populations across the United States, including urban isolated and rural communities. Our mission is to deliver comprehensive, accessible, and affordable care by combining advanced technology with human-centered services.

Why NABR Cares?
Across the U.S., underserved populations face:
- Barriers to access: geographic isolation, transportation issues, and internet connectivity gaps.
- Chronic disease burden: cardiovascular disease remains the #1 cause of death, with early intervention often unavailable to low-income and minority populations.
- Dementia and aging needs: caregiver shortages and fragmented support.
- Social determinants of health: unsafe housing, lack of affordable medical equipment, and limited support for daily living.
Without intervention, these barriers perpetuate cycles of preventable hospitalization, poor outcomes, and economic strain on families and communities. NABR Cares aims to close these gaps with a comprehensive, integrated model that meets patients where they are.
Comprehensive Services
Through Partnerships
Telehealth Facilitation & Home Care
- Platform-agnostic telehealth kits (stethoscope, otoscope, dermatoscope, vitals monitoring, thermometer, BP cuff, scale, pulse oximeter).
- Trained Community Health Workers (CHWs) and phlebotomists conduct in-home visits, lab draws, SDOH assessments, and patient-provider connections.
- Home care services extend to activities of daily living, caregiver relief, and safety monitoring.
Handyman Services
- Support for home safety modifications (grab bars, ramps, fall-prevention adjustments).
- Coordinated by CHWs to ensure safe environments, especially for seniors and disabled patients.
Advanced Diagnostics
- Non-invasive, ten-minute test detecting coronary artery disease, microvascular dysfunction, and early heart failure 8–10 years before symptoms.
- Integrated into annual physicals and urgent visits to reduce preventable ER utilization.
Discount Durable Medical Equipment (DME)
- Direct-cash model for equipment such as walkers, wheelchairs, oxygen concentrators at up to 80% below retail prices.
- Improves affordability and access for low-income patients.
Dementia Care Management
- Evidence-based dementia care coordination aligned with CMS’s GUIDE program.
- Provides navigation, caregiver support, and community resource linkage.
Chronic Care Management (CCM)
- Comprehensive chronic condition management and documentation.
- Eligible for Medicare reimbursement, improving both sustainability and patient outcomes.
Remote Patient Monitoring (RPM)
- Devices (BP cuffs, glucose meters, scales, pulse oximeters) transmit data to providers.
- Early alerts and interventions reduce hospitalizations and complications.
Care Navigation
- 24/7 AI-driven navigator that integrates across EMRs and claims databases.
- Ensures adherence to care plans, closes care gaps, provides multilingual omnichannel support, and escalates to providers as needed.
